Add a Customized Header in Google Forms

1. When you are creating your google form, click on the paint palette icon in the upper right hand corner.
2. Under this menu you can change the theme color, background color or font style.
3. Under the header option (still in the same menu) you will click on the “Choose Image” button.
4. Google does have some images you can pick from but you can add your own customized image. To do this, click “Upload’ and pick your image of choice from your computer.
   • If you want to create an image, make it in powerpoint and click “Save as Picture”, then upload to google forms.
